Refactor LuaAssFile's implementation

Switch to 1-based indices to match Lua rather than converting back and
forth all over the place.

Eliminate a bunch of duplicated code.

Fix a few memory leaks.

Automatically set an undo point at the end of a macro if there are any
uncommited changes.

Originally committed to SVN as r5630.
